My guess was that this all started with a "key party". They were friends, had "key party", luck of the draw and they figure out they like each other's spouse better. No doubt much swapping happened before the conversation was had.
I only know about this because it comes up in at least one of the episodes of Behind the Bastards, but I'll be damned if I can find the one I'm thinking of.
