I would argue deciding to wear this shirt publicly means the person has a mental condition that requires intervention whether sincere or not.
Do you go outside often? Outside is full of people who are dressing specifically to offend however they can.

Report back the numbers when you do.I do that literally every day. It's called people watching and it's a stupid habit I got from my mom.
